On the way, Sandor euthanises a Piper archer who was injured during the Red Wedding, with the aid of Arya. She and the Hound stay beneath the Mountains of the Moon for weeks until they help finish a palisade. The high road to the Eyrie is closed, however, and the Hound decides to take her to Riverrun and her uncle Brynden.
On the way, Clegane and Arya stop off at the inn at the crossroads, where they have the misfortune to encounter Polliver and the Tickler - the Mountain's men - and a squire. There they learn of the siege of Riverrun, and Polliver tries to take Sandor back to his brother. When Clegane insults Polliver and refuses him, a bloody fight erupts in the inn.
The Hound cuts down Polliver, but finds it harder to beat the Tickler until Arya intervenes, killing the Tickler and the squire. Inebriated, injured and weak, Sandor goes in and out of delirium for the remainder of the journey, his wounds becoming infected. Arya eventually decides to abandon the weakened Hound, refusing him the gift of mercy despite him openly pleading for her to kill him. She leaves him to die under a tree beside the Trident. Sandor Clegane's revenge is robbed from him in King's Landing: Prince Oberyn Martell faces off against Gregor Clegane in a trial by combat, and repeatedly wounds him with a poisoned spear - poison which eventually kills him.
Sandor Clegane's whereabouts are unknown for some time. Brienne of Tarth hears stories during her travels searching for Sansa. They say the Hound joined a group of Outlaws and sacked the town of Saltpans in the Riverlands.
People speak of the atrocities he committed. He truly went rogue. He tells Brienne that he was drawn across the river by his cries of pain. Clegane begged the man for mercy, to kill him quickly to end his suffering.
The Elder Brother is sworn not to kill, though, so he gave him a poultice for his wound, some wine, and a cloth to wet his fevered brow. His efforts were too little and too late however, and Sandor Clegane died there in his arms. The Elder Brother buried him there, and laid his helm upon the grave as a marker of his passing, but he admits that was clearly a mistake, as some outlaw seemingly found the helm and used it to hide his identity and insight fear into his victims, as he did at Saltpans.
It is later revealed when Brienne confronts some outlaws outside the Inn at the Crossroads that the fake Hound is actually Rorge, and the outlaws who raided Saltpans are the remnants of the Brave Companions. A fight breaks out, and Brienne kills Rorge, but Biter attacks Brinne and chews the flesh off her cheek. Gendry saves her, but he and some orphans from the Inn take her and her friends captive to attest for her crimes of working with Jaime Lannister to Lady Stoneheart at what appears to be Hollow Hill.
